You've started on the path, and now you can enjoy the journey! Once you decide to engage with Moonstone Hormones, we welcome you to become a member. This has several advantages.

• Continuity of Care - ongoing support with a passionate clinical team who know your history, goals, and response to treatment protocols.

• Predictable Monthly Cost - A simple monthly fee that includes visits, pellet therapy, adjustments and monitoring (and more!) can make hormone care easier to plan for.

• Relationship Based Care - Membership care allows time for questions, education, and shared decision making -- without rushed appointments!

• Coordinated Treatment Plan - Visits, pellet timing, and follow-up are organized in to one cohesive plan rather than isolated appointments.

• Proactive Support, not Reactive Care - Ongoing monitoring helps address concerns early rather than waiting for symptoms to escalate.

• Clear Expectations - The membership structure clearly outlines what's included and what isn't. You'll never have to guess or be surprised by extra fees that you did not know about ahead of time!

What's Included in your Hormone Membership?

There should be no questions about what services you have easy and free access to, and which ones you should expect to come with some additional costs.​​ The good news is, we have included a lot of things in this membership!

• Hormone Management by Nurse Practitioner

• Visits (4 maximum in a 12 month period, not including pellet inserts)

• Ongoing Pellets - Female (every 3-4 Months) and Male (every 5-6 Months)

• Secure Messaging Access to RN

• Dose Adjustments and Boosts as needed

• Refill Coordination and Insurance Prior Authorization for Medications

• Follow-up support

• Education and Resources

• 20% Discount on Biote Supplements

• 20% Discount on Labs

Everything you will need is included, with the exceptions listed below. We were able to get very creative and provide a full comprehensive panel of included services and products. This allows you to have a fully predictable cost each month, so that you can spread out the costs of your hormone replacement therapy.

What's not included?

​

• The cost of any prescriptions from the pharmacy

• Labs (Typically, Labs will be needed twice a year)

• Additional supplements that you may choose as recommended in your plan

• Additional labs as medically indicated

• Imaging* or Referrals* if needed for safety

 

*You may use your insurance or an HSA/FSA card.

What if I don't want to use pellets?

​

The fact is, not everyone will want to use pellets. While the membership model is the only option we currently have for utilizing pellets, we do also offer an Injectable Membership and A la Carte Services for those that need another option. These will also be offered during your consultation if you do not want to proceed with pellet therapy.
